Case Worker - Psychiatric Rehabilitation Program (PRP) New season. New growth. New impact. As spring approaches, it's the perfect time for a fresh start—one that truly matters. At Advocate Support Services, Inc. , our Case Workers help adults with mental health challenges build skills, independence, and community where it matters most: in their everyday lives . We provide mobile, community‑based PRP services across Anne Arundel, Howard, Prince George's, Baltimore City, and Baltimore County , with a standard of at least five (5) meaningful client contacts per month , including telehealth when appropriate. What You'll Do As a Case Worker, you will: Provide rehabilitative visits in community settings (homes and neighborhoods), focusing on: Activities of daily living (ADLs) Coping and emotional regulation skills Social integration and community engagement Budgeting, transportation, and health navigation Meet or exceed the 5‑visit‑per‑month standard through thoughtful, individualized engagement Coordinate care with therapists, prescribers, and community partners, documenting collateral contacts and care planning Support and track person‑centered goals (IRPs) and complete accurate, timely documentation in the EHR Collaborate closely with your Rehabilitation Specialist and supervisor to ensure quality services, compliance, and strong clinical outcomes What You Bring We're looking for someone who has: Experience supporting adults with serious mental illness (SMI) or significant behavioral health needs (community‑based work preferred) Strong communication, organization, and follow‑through in a mobile, field‑based role Comfort with documentation, EHRs, and weekly visit planning, with attention to detail and quality notes A valid driver's license and reliable transportation Ability to work in assigned service areas (AA, HoCo, PG, Baltimore City, Baltimore County) Nice‑to‑Have (Not Required) CPRP certification or progress toward certification Prior PRP experience Bilingual skills Schedule & Work Environment Mobile / field‑based position Flexible scheduling with some Telehealth Weekly supervision and ongoing team collaboration Supportive leadership that values growth, balance, and professional development Ready to Grow With Us?
