Job Description

Position Overview TriSource is seeking a dedicated and compassionate Case Manager to provide direct professional social work and case management services to individuals experiencing homelessness and trauma. Under the supervision of the Director of Case Management, you will play a pivotal role in helping residents eliminate barriers to housing, access vital benefits, and transition into safe, stable, and independent living environments.
Pay Rate:
$28.00 per hour
Job Type/Schedule:
Full-time, temp to perm, Monday-Friday 9am-5pm, on-site (40 hours/week, with occasional evenings/weekends as needed)
Qualifications & Requirements Education:
Master's Degree in Social Work (MSW) from an accredited Council of Social Work Education (CSWE) program is required .
Core Knowledge:
Strong understanding of social work theories, human behavior, crisis intervention, and the management of social/emotional dysfunctions.
Skills:
Excellent interpersonal and written communication skills for writing comprehensive case reports and forms.
Resourcefulness:
Familiarity with local community resources, federal/state/local guidelines, and benefit programs.
Capabilities:
Ability to build strong relationships with clients and external partners, gather/assess complex client information, and provide crisis intervention when necessary. Must be able to pass a background check
Key Responsibilities Housing Stability:
Partner closely with shelter residents to find and secure housing. This includes identifying/vetting properties, meeting with landlords, conducting property inspections, assisting with applications, and coordinating payments.
Resource Linkage:
Assess client strengths and needs to co-create actionable service plans. Provide referrals for benefit entitlement programs, employment services, mental health resources, and healthcare.
Data & Reporting:
Maintain accurate, timely documentation in the HMIS database. Complete client contact sheets, prepare case histories, and monitor resident progress.
Collaboration:
Attend multidisciplinary team meetings, coordinate with external health and welfare agencies, and interpret program policies for residents.
