Job Description

If you are seeking productive ways to give back, this opportunity as a Case Manager for the City of Bangor Public Health and Community Services Division may be for you! The Case Manager (CM) coordinates care to individuals living with
HIV/AIDS.
The CM provides a range of client-centered services to link clients with health care, psychosocial, substance use services, and other community services. Also providing support around timely medical treatment and coordination of continuity of care. The goal with each client is to obtain viral suppression through a multi-team approach. Please see more information regarding this position here: Case Manager Job Description This position is part-time with an anticipated hours per week of 25 to 28 hours, that may change based on business needs. The pay range for this position is $24.45 to $32.90 per hour. Part-time employees accrue Earned Paid Leave time, and are eligible for benefits such as dental, vision, and life insurance; Employee Assistance Program (EAP) benefits, and wellness incentives.
