Accrued Paid Time Off Job Overview:
As a Case Manager, you will assist clients in finding services, programs, and resources to help them gain independence and work towards their established goals; while also providing support, advocacy, referral, and assessment services. Understand each client's diagnosis and recommend services based on individual assessment. Serve as a liaison between clients and all agencies providing services, programs, and community resources. Obtain all necessary releases to provide or receive information as needed. Meet with other service providers to plan and coordinate treatment to ensure the integration of the treatment across the client's home, school, and community.
Requirements:
Must have an MHRT-C certification and bachelor's degree At least 1 year of experience providing social services Clean background check Must be organized, detail-oriented, and able to work independently in the field MAS Community Health is an equal opportunity employer.
