Join our compassionate, multidisciplinary team at Northeast Michigan Community Mental Health Authority (NeMCMHA) and help individuals thrive in nursing homes, adult foster care, and independent living settings. What You'll Do As an Adult Services Case Manager, you'll provide direct treatment, case management, and mental health monitoring services to older adults with serious mental illness and/or intellectual and developmental disabilities. You'll play a key role in: Conducting biopsychosocial assessments and developing person-centered plans Coordinating care and linking individuals to essential community services Monitoring progress and supporting recovery goals Collaborating with nursing home teams to ensure mental health needs are met Providing consultation and training to caregivers and community partners Supporting OBRA Level I and II processes as needed You'll be a trusted advocate, educator, and connector—helping individuals live with dignity, purpose, and support.
