Adult Services Case Manager $2,000 Sign-On Bonus After 180 Days of Full-Time Employment! Join our compassionate, multidisciplinary team at Northeast Michigan Community Mental Health Authority (NeMCMHA) and help individuals thrive in nursing homes, adult foster care, and independent living settings. What You'll Do As an Adult Services Case Manager, you'll provide direct treatment, case management, and mental health monitoring services to older adults with serious mental illness and/or intellectual and developmental disabilities. You'll play a key role in: Conducting biopsychosocial assessments and developing person-centered plans Coordinating care and linking individuals to essential community services Monitoring progress and supporting recovery goals Collaborating with nursing home teams to ensure mental health needs are met Providing consultation and training to caregivers and community partners Supporting OBRA Level I and II processes as needed You'll be a trusted advocate, educator, and connector—helping individuals live with dignity, purpose, and support. What You Bring Bachelor's degree in Social Work or related field, or RN license Current Michigan licensure (LBSW or RN) At least 1 year of experience working with older adults Strong communication skills and a collaborative spirit Functional computer literacy (within 6 months of hire) Lived experience with mental illness, developmental disabilities, or substance use is valued You'll Thrive Here If You Are passionate about person-centered care and recovery Enjoy working in a team-based, mission-driven environment Are comfortable navigating nursing home and community-based settings Want to grow your skills while making a lasting impact About Us NeMCMHA has proudly served Alpena, Alcona, Presque Isle, and Montmorency counties since 1968.
