40 hours per week Position Overview This Case Management Coordinator role is part of an Assertive Community Treatment (ACT) team, providing intensive, community-based behavioral health services to individuals with complex mental health, substance use, and/or developmental challenges. The position is highly field-based and involves working directly with clients in their homes and community settings rather than a traditional office environment. The ACT model is designed to deliver comprehensive, team-driven care, meaning you will collaborate closely with psychiatrists, clinicians, and other professionals to support individuals in achieving stability, independence, and improved quality of life. This role offers a strong balance of clinical work, case management, and real-world impact, making it ideal for professionals who want hands-on involvement in patient outcomes. Compensation LBSW (Licensed): $40/hour (approx. $1,600/week) LLBSW (Limited License): $37/hour (approx. $1,480/week) Overtime paid at 1.5x when applicable Schedule & Contract Details 6-month contract with strong potential for extension or temp-to-hire Monday through Friday day schedule (no nights or weekends) Required participation in on-call rotation for crisis intervention Weekly, structured orientation beginning on a Monday Work Environment & Setting Community-based role requiring local travel Mileage reimbursed at IRS standard rate Clients are primarily adults with serious mental illness and/or substance use disorders Services delivered in homes, apartments, and community locations Team-based model with daily meetings and frequent collaboration Key Responsibilities Provide case management and care coordination for individuals with behavioral health needs Conduct assessments and develop individualized, person-centered service plans Connect clients with community resources, housing, and support services Monitor progress, living situations, and treatment compliance Support medication adherence and treatment engagement Provide crisis intervention and assist with hospital admissions when necessary Collaborate with multidisciplinary ACT team, including psychiatrists and therapists Maintain timely and compliant documentation in accordance with Medicaid standards Core Functions of the Role Frequent in-person interaction with clients in community settings Regular participation in team meetings and care planning discussions Active involvement in relapse prevention, recovery planning, and behavioral support Ongoing evaluation of client needs and adjustment of service plans
Qualifications Required:
LLBSW or LBSW in the State of Michigan Minimum 1 year experience working with individuals with mental health conditions Valid driver's license with reliable transportation and insurance
Preferred:
