Full-Time | Exempt What We Offer Paid Time Off (PTO) & Paid Holidays BC/BS Medical, Dental, and Vision Health Savings & Flexible Spending Accounts Pension Plan Tuition Assistance & Student Loan Repayment Life and Disability Insurance AFLAC Options Position Overview The Case Manager supports individuals with SMI, SED, and/or I/DD by coordinating services that promote independence, community inclusion, and overall well-being. This role involves advocacy, service linkage, and monitoring to ensure continuity of care and prevent service gaps. Key Responsibilities Develop and implement person-centered service plans Link individuals to medical, social, educational, and therapeutic resources Coordinate and monitor services to achieve successful outcomes Provide advocacy and support to reduce barriers to care Participate in emergency on-call rotation Qualifications Bachelor's degree in a human services field Minimum one year of experience in mental health Certification as QMHP, QIDP, and/or CMHP, or Michigan licensure (limited or full) Strong organizational, communication, and problem-solving skills Ability to work flexible hours and travel within service area Valid Michigan driver's license, vehicle insurance, and satisfactory driving record Persons with lived experience in mental illness, developmental disabilities, or substance use disorders are encouraged to apply About Wellvance Wellvance provides state-funded mental health services to residents of Iosco, Ogemaw, and Oscoda counties.
