QUALIFICATIONS
Education:
Minimum of a bachelor's degree in counseling, psychology, social work or related field, must be eligible as a Qualified Mental Health Professional (QMHP) and/or hold/maintain professional licensure associated with degree that is issued by The State of Michigan.
Experience:
One year of experience working with persons with mental illness or similar outreach experience.
Other:
Lived experiences with behavioral health issues are valued. Substance abuse education, certification and/or experience a plus.
SUMMARY OF RESPONSIBILITIES
The ACT Advocate works as a member of the Assertive Community Treatment Team to facilitate individuals maintaining or achieving transition to independent living. Provides services and supports designed to prevent hospitalization, maximize independence, enhance quality of life, foster employment, and encourage social relationships and community inclusion. This position serves adults with high acuity, severe and persistent mental illness, those who may also have co-occurring substance use and complex medical conditions, who require intense services and support, and who without ACT would require more restrictive services and settings.
ESSENTIAL JOB FUNCTIONS
Provides community-based outreach and psychosocial services as identified in the Plan of Service (POS). Responds to emergency needs of individuals assigned to the ACT Program. Participates in the development of Person-Centered-Planning and clinical assessments. Prioritizes services surrounding hospital or residential treatment center discharge. Provides case management functions of assessment, planning, linking, monitoring and advocacy. Attends and participates in clinical meetings, Plan of Service, and ACT or agency meetings. Maintains open and active communication with clients, residential providers, natural supports and community agencies. Coordinates the linking of services to include financial, vocational, housing, transportation and medical with other agencies and professionals. Uses consultative services of agency psychiatrists and other staff. Provides individual and/or group treatment services in accordance with team supervision and credentialing and training. Assists with benefits acquisition and advocacy regarding community resources. Supports an integrated healthcare focus. Participates within the ACT on-call system. Provides complete and timely documentation in accordance with agency policy and procedure and Medicaid requirements. Meets direct and indirect service activity expectations. Performs other related duties as assigned by supervisor.
PHYSICAL REQUIREMENTS
Ability to walk, bend, stand, sit, lift up to 25 pounds with or without assistance, stretch/reach, hear, see, hand/finger dexterity. Position requires physical ability to complete and maintain certification in de-escalation techniques, which will be a training class provided to the job incumbent. OTHER REQUIREMENTS
Must have own phone and internet for communication, reliable access to messages, and multi-factoring capabilities. Have reliable transportation and be able to drive a company vehicle. Must also possess a valid Michigan driver's license and have a satisfactory driving record. Ability to work within guidelines of the code of conduct, regulatory compliance plan and personnel policies. Supports a philosophy of service delivery that is recovery-based, person centered and culturally competent. Must have working knowledge of general office procedures and type at least sixty (60) words per minute. Ability to word-process on the computer and process written and dictated reports. Must be current in all applicable trainings; provided by NCCMH. Must be able to maintain strict confidentiality both during and after work hours.
