Royal Oak Autism Connections Job Summary :
The Autism Navigator is responsible for providing guidance and support to individuals with autism and their families by connecting them with resources, services, and support systems throughout their service plan. The Navigator helps individuals and families provide screening for social determinants of health, referrals to service providers, educational support, and community resources.
Responsibilities:
Assess families whose children are in the ABA program at intake and at specified intervals for social determinants of health using a standardized screening tool such as PRAPARE. Referral and follow up with needed services as identified by the family and the social determinant of health screening related to food, housing supports, school advocacy, transportation, etc. - that may be barriers to participation in ABA treatment. Assist with coordination of health care, specialty services such as speech and occupational therapy and behavioral health care services and/or referrals to needed services. Assist with coordination of educational services and resources. Prepare data reports for services provided to families related to identified needs, referrals provided and service engagement. Provide excellent customer service. Other duties as assigned. Qualifications Bachelor's degree in related field, such as Social Work, Psychology or Applied Behavior Analysis or certification as a Community Health Worker. Valid Michigan driver's license and insurance, use of own vehicle. Working Conditions Generally, an office environment; Services provided during Clinic office hours Monday through Friday Ability to travel and attend meetings and functions at various hours Job travel, utilizing personal vehicle with mileage This description is intended to describe the type and level of work being performed by a person assigned to this job.
