Care and Share Case Manager Job Description This is a full-time position, although part time will be considered at applicants request. Days and Hours of work are generally Monday-Friday, 8 hours per day, 40 hours per week. How to

Position Purpose:
The Care and Share mission is to provide temporary housing and meals for adults and families who are homeless; as well as provide support and services to assist them to re-enter their chosen community in their own residence. Care and Share Case Managers provide case management services to Care and Share residents to support them in their search for permanent housing. Services are designed to provide an opportunity for the resident to obtain greater self-sufficiency.
Services include:
resident advocacy, identifying resident personal goals, guidance and assistance with strategies to achieve resident goals, and, referrals to appropriate agencies and community resources in support of meeting resident goals.
Responsibilities:
Ÿ Abide by and comply with the rules of confidentiality, Data Privacy Act, mandatory reporting laws, HIPPA requirements, social work ethics and organizational policies and procedures.
  • Conduct intake and detailed assessment of prospective program participants. Assist residents to identify and overcome barriers to self-sufficiency.
  • Report and log activities and update essential occurrences.

Support resident actions and activities that promote housing stability. Act as an advocate for residents as appropriate when requested by a resident while maintaining a commitment to empower residents to solve their own problems. Input data into the Homeless Management Information System (HMIS) in a timely manner for reporting and funding purposes.
  • Develop and maintain working relationships with other agencies that provide services to residents.
  • Address resident noncompliance with housing program rules and guidelines to uphold a safe and supportive housing environment.
  • Work closely within the Case Management team to ensure productivity while accepting referrals and maintaining a case load that reflects the number of residents Care and Share can serve at a given time. Identify when crisis assistance is needed by a resident or prospective resident and make the appropriate referral for crisis services.
  • Assist in the safe evacuation of all persons during an emergency or disaster situation.
  • Attend meetings, trainings, conferences related to fulfilling and elevating the Case Manager role.
  • Perform other duties as assigned.
Required Education and Experience 1. Bachelor's Degree in Social Work or similar degree/relevant experience required. Minnesota Licensed Social Worker desired. 2. Previous experience working with individuals who are homeless preferred, including experience working with issues commonly experienced by individuals who are homeless such as trauma, mental health concerns and substance abuse. 3. Must have the ability to collaborate within the Case Management team and with other organizations while advocating for residents seeking housing. 4. Must have the ability to work with a highly diverse adult at

Must have the ability to build rapport with people from different backgrounds and cultures. 6. Must have strong documentation skills 7. Must have or be able to develop knowledge of community resources and service providers in area counties 8. Must adhere to all organizational and programmatic policies and procedures 9. Must have computer skills sufficient for navigating data base programs and MS Office 10.
