A Brief Overview Serves a core member of the collaborative care team. Responsible to plan, organize and implement direct service to patients and families. The Behavioral Health Case Worker is responsible for supporting and coordinating the mental and physical health care of patients on their assigned case load. Maintains contact with referring agencies and related people, and with community resources used for referral. Assists patients and families in assessing financial resources. Works closely with care team and community stakeholders to provide best patient care and promotion of Avera. What you will do Serves as a resource to members of the interdisciplinary team in terms of additional continuing care options. Interviews patient and family members, if applicable, to obtain information about home environment, family relationships, health history, and patient's personality traits. Provides necessary referrals for clinically indicated service needs. Assists patients and families to understand, accept, and follow medical recommendations. Performs a variety of services such as advising on social problems, providing patient education, and arranging for continued aftercare. Provides brief behavioral interventions using evidenced-based psychosocial treatments (e.g. problem-solving treatment or behavioral activation) as clinically indicated. Prepares and updates records for each patient. Prepares required documentation of each case and maintains records and reports as required. Serves as a representative on the inter-disciplinary care team. Evaluates data gathered in terms of the medical plan of treatment, available social service programs, and plans a pertinent therapy program that will provide maximum benefits for each patient. Completes assessments in a timely fashion. Assists with staff/student on the job training/orientation. Assumes other responsibilities with BHS projects and community activities as assigned. Promotes Avera at community events, and maintains positive relationships with community partners. Essential Qualifications The individual must be able to work the hours specified. Required Education, License/Certification, or Work Experience:
