Non-Exempt Position Summary Semcac is seeking a compassionate and driven Outreach Services Case Manager to provide direct support, resources, and advocacy to individuals and families in need. In this role, you will work closely with community members to assess needs, determine eligibility for services, and connect them with essential programs and resources that promote stability and self-sufficiency. This position plays a critical role in strengthening the community by empowering individuals to overcome economic challenges and achieve independence. Key Responsibilities Provide intake, application assistance, and eligibility determination for programs and services Assess client needs and develop individualized service plans Offer information, referrals, and follow-up support to clients Manage a caseload, ensuring timely documentation and reporting Build and maintain relationships with community partners and service providers Respond to calls, emails, and walk-in clients, including crisis situations Advocate for low-income individuals and families Coordinate and support housing-related services (e.g., homeless prevention, transitional housing, motel vouchers) Work with landlords, housing agencies, and community resources to stabilize households Facilitate or participate in provider meetings and community collaborations Conduct outreach to identify and inform eligible individuals about available services Enter and maintain accurate client data in agency systems (e.g., HMIS) May supervise volunteers or support staff and assist with food shelf operations if applicable Represent SEMCAC through community presentations and outreach efforts
Education & Experience:
Associate degree (or higher) in Human Services or related field preferred 2-4 years of related experience, or equivalent combination of education and experience
Skills & Abilities:

Pay:
$21.08 - $23.45 per hour
