Community Support Professional (Children/Youth)

Qualifications:

Must possess at least a bachelor's degree from an accredited educational institution in the field of psychology, counseling, social work or a related field. Equivalent experience may be substituted on the basis of one (1) year of experience for each year of required education training (for Community Support Services only).

Duties:

Conduct individual rehabilitative sessions at the office, in client's home or in the community. Participate in the development and implementation of appropriate treatment plans. Maintain an accurate and updated file on each client. Submit appropriate records for billing. Refer clients to community resources for financial, vocational, nutritional, medical, spiritual, transportation, and housing assistance. Comply with all laws, regulations and policies concerning the treatment of clients. Serve on committees as appointed. Provide billable services appropriate to the clients' needs while meeting the production expectation for the position. Facilitate and participate in continuity of care of clients served. Other duties as assigned.
