This position is responsible for helping clients identify barriers to housing stability, employment, healthcare, mental health support, and self-sufficiency while developing individualized service plans that promote long-term success. The Case Manager works collaboratively with shelter staff, community organizations, healthcare providers, housing agencies, and social service partners to ensure clients receive comprehensive, trauma-informed care in a safe and respectful environment. Client Support and Case Management Conduct client intake assessments, screenings, and needs evaluations. Develop individualized service plans with measurable goals and timelines. Provide ongoing case management services, including crisis intervention and problem-solving support. Assist clients in obtaining identification documents, benefits, healthcare, employment resources, transportation, and housing applications. Support clients in accessing mental health, substance use treatment, and medical services when appropriate. Monitor client progress and update case plans regularly. Maintain accurate, timely, and confidential client records and documentation. Advocate on behalf of clients with landlords, employers, agencies, and community partners. Facilitate referrals to community resources and follow up on outcomes. Housing and Stability Services Assist clients with housing navigation and placement. Coordinate with local housing authorities, landlords, and supportive housing providers. Educate clients on budgeting, life skills, tenancy responsibilities, and independent living. Support rapid rehousing and homelessness prevention efforts when applicable. Shelter Operations Support Maintain a safe, welcoming, and trauma-informed environment for all clients. Participate in team meetings, case reviews, and interdisciplinary collaboration. Support shelter policies and procedures, including safety and emergency protocols. Assist with conflict resolution and de-escalation when needed. Participate in data collection, reporting, and program evaluation activities. Education and Experience Bachelor's degree in Social Work, Human Services, Psychology, Counseling, or a related field preferred. Equivalent combination of education and experience may be considered. Minimum of 1-2 years of experience in case management, homelessness services, behavioral health, social services, or related field preferred. Knowledge, Skills, and Abilities Strong understanding of homelessness, trauma-informed care, and community resources. Ability to work effectively with individuals from diverse backgrounds and experiences. Excellent communication, interpersonal, and organizational skills. Ability to manage multiple cases and maintain detailed documentation. Knowledge of crisis intervention and conflict de-escalation techniques. Proficiency in Microsoft Office and case management databases. Ability to maintain professional boundaries and confidentiality. Strong problem-solving and advocacy skills. Physical and Work Environment Requirements Ability to sit, stand, walk, and move throughout the shelter environment. Ability to occasionally lift up to 25 pounds. Ability to work in a fast-paced environment with individuals experiencing crisis situations. Flexibility to work evenings or weekends as needed. Preferred Qualifications Experience working in housing-focused programs or community support services. Knowledge of local community resources and coordinated entry systems. CPR/First Aid certification. Bilingual skills are a plus.
