Job Duties:
Responsible for coordinating and conducting in-person visits with the Office's clients, monitoring client placements, and obtaining client information with staff members on financial and legal matters, with community health workers on medical and mental health issues, with state and federal agencies on entitlement questions, and with facilities to assure appropriate living arrangements or placement for wards. Assists assigned case managers by supplementing in-person client visits; assisting with completion of client documents, including annual treatment plans and documents, admission agreements, hospice agreements, and similar documents; and gathering information for public benefit applications.
Minimum Qualifications:
Bachelor's Degree preferred but may substitute experience working with adults who have chronic mental illness, substance abuse disorders, physical, cognitive, or intellectual disabilities, behavior, or personality disorders. Must have experience with Microsoft Word and Outlook. Must have a valid Driver's License. Must submit to and pass a pre-employment background check and drug screen.
