Job Description

Pay:
Base Wage starting at $20.45; increased based on experience
Incentive Eligible Position:
Yes Essential Functions:
Completes psychosocial assessments with each patient to guide the treatment process and assist in establishing appropriate follow up care. Collects and analyzes data from various sources to evaluate programs and determine need for additional educational programs. Delivers behavioral health education services. Develops educational materials such as audiovisual aids, articles, brochures, fact sheets and public service announcements promoting behavioral health topics. Maintain patient confidentiality in treatment plans and reporting as appropriate. Understand and abides by HIPPA guidelines. Maintain complete and accurate records on each patient. Inform and educate the patient and family member involved in ongoing treatment plans for the patient. Informs all parties involved on the processes that are being used to treat the patient. Sets up and attends meetings with providers and patients as needed. Receives documentation from providers and follows up with patients regarding their treatment plan. Serves as a liaison between the patient and the mental health provider. Provides outpatient mental health resources to patients and assists in establishing follow up appointments. Other duties as assigned.
Skills Abilities:
Ability to communicate clearly and effectively with all levels. Ability to complete competency for nursing skills. Ability to perform simple mathematical equations. Ability to prioritize and complete multiple tasks simultaneously. Effective oral and written communication skills.
Working Conditions:
Exposure to distressed patients, families or visitors. Possible exposure to inside environmental conditions.
Physical Demands:
Requires standing, sitting or walking for up to 12 hours or longer per day. The ability to lift up to 50 lbs.
Qualifications:
Must maintain a telephone and adhere to appropriate dress code. No licensure required. Bachelor's Degree a human services field of study which includes a field practice involving supervised psycho-social assessment, planning and referral of individuals with health and/or social problems is required.
Preferred Qualifications:
Experience in a hospital or long-term care setting preferred. Other experience in related fields helpful. NRMC is adhering to the CMS Omnibus
