HollomanCare is seeking a dedicated and compassionate Case Manager to join our healthcare team. We support Medicaid recipients in hiring and managing their own caregivers, empowering them to maintain independence in their homes. Key Responsibilities:

Manage a caseload of approximately 30-50 clients and their caregivers Monitor utilization and care plans to ensure compliance with program guidelines Conduct monthly visits with participants (both in-person and via phone) Conduct yearly vists with clients in their homes Must be comfortable in both an office setting as well as a field (community) setting Assist participants in accessing community resources to promote independence Maintain accurate and timely documentation in compliance with HIPAA regulations Facilitate communication between participants, families, and care teams Participate in team meetings to review progress and coordinate services

Qualifications:

Background in healthcare, social work, or a related field required Must be outgoing have the ability to self-motivate Knowledge of HIPAA regulations required Strong organizational and communication skills Ability to manage multiple clients and priorities effectively Prior experience in office, case management, or supervisory roles preferred Why Join HollomanCare?
