Elwyn seeks a Forensic Navigator who serves as the primary point of contact for individuals in a community-based competency restoration program, providing care coordination to support progress toward competency. Responsibilities include assisting with scheduling, transportation, and system navigation; addressing barriers to engagement; developing individualized service plans; and facilitating communication between courts, attorneys, providers, and community agencies. The role tracks client progress, delivers status updates, maintains accurate documentation, and provides administrative support to the Forensic Team. The position also requires maintaining required certifications, adhering to all policies and procedures, and cooperating with audits, inspections, and investigations.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Serve as the primary point of contact for individuals participating in community-based competency restoration program Assist clients with appointment scheduling, transportation arrangements, and system navigation Identify and address barriers to engagement, including social determinants of health Develop and implement individualized service plans in collaboration with clinical and legal teams Provide care coordination to support participants in gaining competency including facilitating mental health treatment, medication management, and psychoeducational interventions Facilitate communication between courts, attorneys, treatment providers, and community agencies Track client progress toward competency restoration and provide regular status updates to stakeholders Maintain accurate, timely documentation in accordance with program, legal, and regulatory requirements Provide administrative support to Forensic Team as needed (e.g. submit medical records requests, data entry, submit invoices and reports) Maintains current state-mandated training and certifications Follows all current policies and procedures of Elwyn North Carolina Cooperates with Elwyn North Carolina and the State of North Carolina Department of Human Services, or any other authorized investigating body during an inspection, an inquiry, or investigation
QUALIFICATIONS
Bachelor's degree in human services or an associate's degree with 4 years of experience. Previous experience working within and managing a unionized workforce preferred Non-profit human service experience highly preferred Demonstrated ability to work effectively as part of a team Ability to work in a fast-paced environment and manage and prioritize multiple, often competing, priorities Demonstrated strong attention to detail Demonstrated strong time management and organizational skills Demonstrated ability to anticipate future needed changes or identify problem areas and take effective actions Must be able to perform CPR and crisis intervention using agency-trained protocols Must possess excellent customer interaction, collaboration, presentation, and written and verbal communication Demonstrated (basic, intermediate, advanced) experience with Microsoft Office applications, including Word, Excel, Outlook, and PowerPoint; Access, Publisher and report-writer experience preferred (Experience using/knowledge of) Electronic Health Record (EHR) / Electronic Medical Record (EMR) Must have current, valid driver's license in state of residence, three (3) years driving experience in the United States, and acceptable driving record
