First Fruit Ministries is seeking a compassionate, resilient Outreach Case Manager to lead our street outreach and crisis response efforts. We are a mission-driven organization in Southeastern North Carolina, sharing God's unconditional love by serving those experiencing homelessness or human trafficking. As the Outreach Case Manager, you will be on the front lines—combining professional trauma-informed care with compassionate ministry. You'll build trust-based relationships in the field, coordinate emergency shelter through The Guest House , and provide the advocacy needed to help women move toward safety, stabilization, and housing. Benefit from 16+ paid holidays annually to ensure you stay refreshed for the mission. Job Summary The Outreach Case Manager is responsible for field-based ministry and social services, driving the success of our street outreach and Guest House operations.
Key focuses include:
Street Outreach:
Building trust with individuals in encampments, hotels, and community settings.
Crisis Response:
Coordinating intake and stabilization for The Guest House emergency shelter.
Advocacy & Case Management:
Helping participants navigate pathways toward treatment, healthcare, and permanent housing.
Key Responsibilities:
Direct Engagement:
Conduct targeted street outreach to identify victims of trafficking and those in crisis.
Shelter Operations:
Support the women's drop-in day shelter and coordinate emergency Guest House intakes.
Case Management:
Maintain accurate documentation in EmpowerDB and HMIS while managing housing transitions.
Crisis Coordination:
Participate in a rotating on-call response to support survivors in immediate need.
Community Partnership:

Qualifications Heart:
A strong Christian faith, spiritual maturity, and a commitment to our Holy-Spirit-led mission.
Gender Requirement:
Female applicants only (Bona Fide Occupational Qualification due to the nature of shelter and survivor services).
Education:
Bachelor's degree in Social Work, Counseling, Ministry, or related field required; Master's degree preferred.
Experience:
3+ years in outreach, trafficking response, crisis intervention, or homelessness services.
Skills:
Knowledge of trauma-informed care and survivor-centered practices; proficiency in data reporting (HMIS/EmpowerDB preferred); excellent advocacy and communication skills.
Logistics:
Valid driver's license, reliable transportation, and proof of insurance (must be able to transport clients).
