Job Description

The SquareOne Navigator is a welcoming face of the SquareOne Family Resource Center, serving as a point of contact for families seeking support and connection to community resources. Working under the supervision of the SquareOne Manager, the Navigator assists with strength-based assessments, provides input in developing individualized action plans, and provides warm handoffs to community partners. This role is grounded in trauma-informed, motivational interviewing principles and is committed to equity, inclusion, and the removal of barriers to services. SquareOne Resource Center is open 24/7/365 to families in need. What We Ask Client Services Serve as the welcoming point of contact for all families arriving at the SquareOne Family Resource Center Assist with conducting strengths-based assessments using approved tools, applying motivational interviewing and trauma-informed principles Identify and respond to immediate needs, connecting families to emergency supports as needed Record Keeping/Case Documentation Utilize platforms such as "Find Help" to document all family interactions accurately and in a timely manner Maintain confidential and organized client files in accordance with agency policies and procedures Uphold strict confidentiality standards and comply with HIPAA regulations Complete required data entry and outcome tracking to support program reporting to funders Participate in the agency's PQI (Process Quality Improvement) process. Community Relationships Help maintain collaborative relationships with schools, healthcare providers, community-based organizations, and law enforcement May be asked to participate in community meetings, coalitions, and outreach events. Work collaboratively with partner agencies to uphold shared protocols and ensure consistency in navigation and referral processes Assist the Coordinator with monitoring family experiences to identify patterns, gaps, and barriers within the local system of care and communicate findings to the program coordinator Agency Involvement/Professional Growth and Development Take responsibility for continued learning and for professional growth and development. (e.g., attending workshops, completing research, professional reading, etc.) Attend required meetings. Use opportunities offered by the agency to strengthen agency or professional knowledge and skills. Participate in agency committees as assigned. Participate in agency PQI activities. Program Support Assist in the coordination of supplemental services, including housing stabilization, safe sleep, youth enrichment, and transportation programs Help organize family-friendly events and community engagement opportunities, ensuring accessibility and inclusion Contribute to continuous quality improvement efforts by participating in staff meetings, trainings, and performance reviews Support the Coordinator in identifying program needs and opportunities to enhance service delivery Interpersonal Relationships Interact in a way that is supportive of clients, the agency, and the profession. Demonstrate the ability to build rapport with individuals from diverse backgrounds. Follow the Family Service Code of Ethics and treat all clients with dignity and respect. Cultural Sensitivity Embrace and display sensitivity relative to the cultural and socioeconomic characteristics of the agency's service population. REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES
Knowledge of community resources and case management best practices Ability to work with a variety of individuals from diverse backgrounds. Ability to work cooperatively and professionally with community partners and representatives from other agencies. Ability to work independently Ability to drive and provide proof of driver's license and auto insurance.
