Job Description

The case manager is responsible for assessment of and service delivery to inmates. The case manager facilitates groups and provides individual case management for residents. The case manager is responsible for ongoing support, follow-up and strict supervision according to program procedures. Develop and maintains professional relationships with community treatment providers and community employers. Informs a potential employer about benefits of employing inmates: Work Opportunity Tax Credit. Facilitates education sessions as directed by the Program Director, to include job readiness and life skills Develops and monitors individual service plans for inmates by making appropriate referrals for medication management, housing, clothing, food, and etcetera prior to discharge. Acts as a liaison to community members and organizations representing the facility in a professional manner Keeps the Program Director informed of inmate progress throughout the person's incarceration Establish an inventory of available housing options for released inmates. Collaborates with program team on a weekly basis to ensure that inmate goals are being met Maintain inmate records according to program procedures and facility policies. Completes all required paperwork in a timely manner to assure a smooth transition through the facility from intake to release. Any other duties deemed necessary by the Program Director The candidate for this position shall possess a Bachelor's Degree in education, rehabilitation counseling or human services from an accredited college or university. The successful candidate will have a thorough knowledge of the principles of correctional institution operations, knowledge of criminal codes, knowledge of facility rules and regulations, skills training and instruction, have the ability to use computers, manage difficult people under stress and is able to communicate effectively. The candidate must possess a current New Hampshire driver's license and pass a physical examination and drug screen. The candidate must be capable of social and professional interaction on varying levels of cognitive behaviors to include educational, literacy, cultural, developmental disabilities, health and biomedical, as well as community needs such as housing and dependent family situations.
