Job Description

Job Summary The Case Management Supervisor oversees the Agency's call center and case management services as part of the Information and Referral (I&R) Team under the direction of the Resource Center Director. The Manager has supervisory responsibilities for all Resource Center Specialists who carry BH caseloads. The I&R Team links individuals who need assistance with appropriate service providers and/or supplies. Services may include, but are not limited to, financial assistance, disability related services (Social Security and Medicaid), medical programs, rehabilitative services, vocational rehabilitation, legal assistance, housing assistance, support and educational based support groups, disaster relief, food pantries, home delivered meals, in home assistance or other organizations that empower individuals to live independently within their community.
Qualifications:
  • Bachelor's Degree required in human services or related field such as Social Work, Psychology, or Education
  • Minimum of three years of social work or case management experience, preferably in the areas of aging, disabilities, community health, nursing home or hospital discharge planning
  • Alliance of Information Referral Specialist (AIRS) certification
  • Experience in customer relations, call management, information and referral or related field that includes phone skills and preliminary assessment and triage ability of contacts
  • Experience working with persons with disabilities preferred
  • Experience/working knowledge of basic health insurance principles preferred
  • Ability to communicate effectively both orally and in writing
  • Ability to plan and perform diversified duties
  • Must be able to exercise independent judgment and use discretion in the performance of assigned duties
  • Ability to utilize technology provided to perform requirements of the job
  • Valid driver's license, proof of adequate automobile liability insurance and reliable transportation required
  • Ability to travel in state and occasional travel out of state for conference, trainings, and meetings
  • Ability to work occasional evening hours
Pay:
From $60,000.00 per year
