Job Description

Job Description Insight Global is looking for a Lead Case Manager for one of their top social services clients in South Jersey. This role focuses on providing direct counseling and case management services to high-risk, unaccompanied youth while supporting their journey toward safe reunification with appropriate sponsors. On a day-to-day basis, the individual manages an assigned caseload overseeing 4 caseload, develops and updates personalized reunification plans, and ensures each youth receives comprehensive, integrated services, including legal, medical, and trauma-informed care. They regularly meet with youth, at minimum weekly, and coordinate with internal teams, external agencies, and sponsors to monitor progress and move cases toward timely release, typically within 30 days. The position also involves supervising 4 case managers to maintain appropriate staff-to-youth ratios, ensuring compliance with ORR systems and documentation standards, and tracking all case activity through required databases such as the UC Portal. Daily responsibilities include transporting youth to medical, educational, recreational, and other appointments, as well as providing advocacy with courts, agencies, and family members. The role requires detailed documentation, including progress notes, incident reports, and data tracking, while also supporting crisis intervention and maintaining 24-hour program coverage through rotating on-call responsibilities. In addition to direct service work, the individual collaborates with supervisors and committees, participates in ongoing training and professional development, and contributes to a team environment that aligns with the agency's mission and values.
