Job Description

Friends of Cyrus provides services to adults with Developmental Disabilities.

The Case Manager, based in Edison, NJ, will be in charge of managing all Medicaid, Housing & Social Security Related documentation for clients in our Group Homes.
RESPONSIBILITIES
Review of Medicaid, Housing, Social Security applications and other correspondence relating to clients we serve. Collect all necessary documents to apply for Medicaid Maintain all documentation to maintain Medicaid Eligibility Secure and complete appropriate forms in accordance with the established procedure Follow up to ensure that all applications are approved Liaison with Government Agencies, including but not limited to County Social Service Department & NJ State Department of Human Services Review, maintain & file all relevant paperwork Attend all meetings as required by management Identify and maintain a professional relationship with any community resources that may be of assistance to the program and/or consumer Assure that all activities are within guidelines set forth by Friends of Cyrus. Develop and maintain a good working relationship with the consumer families to promote communication, problem-solving, and service consistency. Perform any other duties as required or assigned.
