Part Time Evening Housing Case Manager The Gateway Family YMCA, Elizabeth Branch Elizabeth, NJ Job Details Part-time $18 - $21 an hour 1 day ago Qualifications Care documentation Mental health services Conflict management Direct patient care in social services case management Group counseling Full Job Description Job description: This position ensures that the residents in the YMCA stabilize their life and functions at maximum efficiency and courtesy to fully support the resident's objectives of the YMCA To help client clarify their goals and objectives. To significantly improve each resident's ability to obtain and maintain permanent housing To increase the client's motivation to remain employed and long-term satisfaction with their work environment. The achievement of established program outcomes The achievement of quality and timely promotional and communications materials. accurate and timely submission of reports and documents to Director of Social Services. Develop relationships with representatives in other agencies to support individuals and families in attaining services such as permanent housing, mental and physical health care, substance abuse treatment, financial assistance, legal issues, etc. Experience with grant compliance. Knowledge of Union County Social Service providers. Provide individual and group counseling for residents. Coordinate, prepare and maintain required charting and documentation in a timely and thorough manner. Develop individualized success plans with mutually agreed upon measurable goals and objectives. Adhere to all client confidentiality requirements and standards. Ability to effectively resolve conflict and cope with crisis situations. Strong and timely documentation and assessment skill
