In this role, you will coordinate services, advocate for client needs, and facilitate access to resources that promote stability, independence, and well-being. Your energetic approach will help clients overcome barriers, develop life skills, and achieve long-term housing success. This position offers an exciting opportunity to make a meaningful difference in the lives of vulnerable populations through proactive engagement and resource management.
ESSENTIAL JOB DUTIES
Responsible for ensuring the following services are provided to clients: case management, advocacy, and referral. Responsible for overall management and coordination of Supportive Housing program. Provides Director of Residential Services with information on potential problems and assists in the development of solutions and/or innovative approaches to problems. Exhibits sensitivity and concern when interacting with residents. Responsible for helping to create an empowering and safe living environment for clients. Provides and maintains accurate client records and reports. Implements the Supportive Housing program in collaboration with the Director of Residential Services.
Provides the following direct services:
intake/exit interviews, weekly meetings, referrals, advocacy, and development of case management plans for each client. Maintains contact with community agencies to coordinate the delivery of services. Refers clients to appropriate community resources for services not covered by YWCA Union County. Accompany clients to community resource appointments as needed. Responsible for following up with clients who have missed appointments and documenting contacts in the case record. Qualifications Must have a BA or BSW with prior case management experience. Knowledge of computer programs including but not limited to Microsoft Office Suite (Word, Excel, Power Point, Outlook). Valid Driver's License and reliable transportation. Maintain flexibility in schedule to meet client needs. Be available for some evenings and weekends. Residential experience, experience in domestic violence preferred. Bi-lingual Spanish skills preferred. Knowledge and sensitivity to issues of domestic violence, substance abuse and homelessness and a commitment to women's empowerment and the eradication of racism. Excellent verbal, written, and public speaking skills.
Pay:
$54,000.00 - $55,000.00 per year
