Full Time Guttenberg, NJ Posted 5 days ago $55,000 USD NHCAC The Housing Case Manager supports individuals and families experiencing homelessness in securing and maintaining permanent, affordable housing. Through comprehensive case management, assesses participant needs, identifies and addresses barriers to housing, and develops individualized housing stabilization plans that promote long-term self-sufficiency. This role works collaboratively with internal staff and community partners to connect participants to essential resources, monitor progress, and ensure program compliance. Maintains accurate documentation and reporting in accordance with funding and regulatory requirements.Qualifications:
Bachelor's degree in Social Work, Counseling, or a related field, or equivalent experience (minimum 5 years in social services) Experience working with vulnerable populations, preferably individuals experiencing homelessness Strong knowledge of community resources and housing programs Excellent interpersonal, organizational, and case management skills. Bi-lingual English/Spanish preferred. Must be fully vaccinated againstCOVID-19
