Full-Time Case Manager (Entry-Level) Reports to:
Executive Director Position Overview:
We are seeking a dedicated and compassionate Full-Time Entry
  • Level Case Manager to join our team at New Hope Behavioral Center.
In this role, you will be responsible for supporting clients with mental health challenges by helping them navigate services, create individualized treatment plans, and ensure they receive the necessary resources for recovery and stability. This position requires the ability to work closely with clients, their families, and multidisciplinary teams to provide comprehensive case management services.
Key Responsibilities:
Develop and implement Individualized Recovery Plans (IRPs) for clients, in collaboration with the client and treatment team. Conduct regular assessments to evaluate client needs, progress, and any changes in mental health status. Coordinate and facilitate access to community resources, such as housing, employment support, medical care, and mental health services. Maintain consistent communication with clients to ensure they are receiving appropriate services and support. Provide advocacy on behalf of clients to ensure access to services and resources that support their mental health and overall well-being. Complete daily progress notes documenting client interactions, progress toward treatment goals, and any critical developments. Review and update IRPs quarterly, making adjustments based on client progress, new challenges, or changing needs. Monitor clients' adherence to treatment plans and follow up to ensure they are attending therapy, appointments, and engaging in other necessary services. Work closely with mental health professionals, healthcare providers, and community agencies to coordinate care and ensure comprehensive support. Maintain accurate, up-to-date client records, ensuring confidentiality and compliance with organizational policies, legal requirements, and ethical standards.
Qualifications:
Bachelor's degree in psychology, social work, mental health counseling, or a related field. Associates degree with 2 years of relevant experience. Previous experience in case management, social work, or mental health services is preferred. Strong interpersonal skills with the ability to build trust and rapport with clients and their families. Excellent organizational and time management skills to handle multiple cases and priorities. Knowledge of community resources, mental health services, and local support networks. Ability to complete accurate documentation, including progress notes, assessments, and treatment plans. Empathy, active listening skills, and a non-judgmental approach to working with clients facing mental health challenges. Must be willing to undergo background checks and comply with organizational and regulatory standards.
