Job Description

JOB SUMMARY
(This position is located in Atlantic County, NJ) Jewish Family Service's Jail Re-Entry Program provides discharge planning for inmates preparing for the transition from incarceration to society. The duty of the Re-Entry Case Manager is to provide an outreach service inside the Atlantic County Justice Facility specifically by engaging, providing case management, advocacy, and participating in release planning. The Re-entry Case Manager carries a caseload of incarcerated clients, and works with a multitude of team members to carry out the utmost appropriate discharge plan tailored to each client. Team members include, and are not limited to; a GED Specialist, Vocational Specialist, Mental Health Justice Outreach Specialist, and Licensed/Certified Drug and Alcohol Counselors. The Re-entry Case Manager refers clients to outside agencies and also works closely with other community services and partners. The position ensures contract commitments under professional, county, and agency standards. Knowledge of local resources and basic knowledge of the criminal justice system is a plus. Individual group and family intervention. Outreach case management and support Referral, advocacy for linkage to essential services after analysis and evaluation of consumer needs and goals Provides or arranges client transportation. Provides skills development training, coupled with or based upon an assessment of development and progress. Organizes and supports recreational and/or socialization activities.
Working Conditions :
Work is performed at the Atlantic County jail. Ability to pass background check is necessary for this position. Education/Experience Ability to engage, assesses individuals in a correctional institution, abiding by policies of the institution. Bachelor's in Social Work, Psychology, Behavioral Health, Counseling, Psychiatric Rehabilitation, or Criminal Justice. One year of experience in an MH setting is required for case managers with a Bachelor's Degree.
