Since 1995, BHCC has provided comprehensive extended care for individuals recovering from illness or injury, along with a nationally recognized program delivering specialized long-term care for people living with
HIV/AIDS.
BHCC's holistic model brings together medical, social, and spiritual support to help residents heal, regain independence, and live with dignity. BHCC is New Jersey's only specialized long-term care facility for people living with HIV/AIDS, and that our
HIV/AIDS
program is nationally recognized. BHCC emphasizes a comprehensive/holistic approach that supports "medical, social, and spiritual" needs, with the goal of empowering residents to live independently and with dignity . Position summary The primary focus of this position is to assist residents with access to primary medical care and medications, identify and remove barriers to medical care, and ensure adherence to a prescribed treatment plan. Maintain accurate and up to date records, in addition to, assist in the planning, organizing, implementing, evaluating, and directing the overall operation of the Social Services Department in accordance with current applicable federal, state and local standards, guidelines and regulations; and within the companies established policies and procedures. A significant number of our residents are People Living with
HIV/AIDS
(PLWHA). Key Responsibilities Provide comprehensive case management services to clients with medical, mental health, and substance abuse needs Conduct client assessments and develop individualized care plans Coordinate medical care, behavioral health services, housing, and community resource referrals Support clients participating in Medication Assisted Treatment (MAT) programs Provide individual and group counseling/support sessions as approriate Maintain accurate and timely documentation in compliance with agency and Ryan White program requirements Monitor client progress and follow up on treatment adherence and care coordination Collaborate with interdisciplinary teams, healthcare providers, and community organizations Assist clients with crisis intervention and advocacy services when needed Required Qualifications Bachelor's degree in Social work or any related field required. Experience working with
HIV/AIDS
populations required Knowledge of Ryan White program guidelines and services preferred Experience in substance abuse treatment and/or Medication Assistant Therapy(MAT) Experience providing group and individual counseling. Strong understanding of mental health conditions and community-based support services Excellent communication, organizational and documentation skills Ability to work independently and collaboratively with a multidisciplinary team Bilingual candidates are encouraged to apply
