In addition, UBHC is the primary mental health training resource for the New Jersey departments of Human Services, Children and Families, and Corrections, delivering 16,000 trainings each year. Posting Summary Rutgers, The State University of New Jersey, is seeking a Habilitation Counselor for Justice-Involved Services for the Community Based Services within Rutgers University Behavioral Health Care. The Habilitation Counselor provides community-based case management and support services for individuals served to include assessment, service / recovery planning, skill-building, linkage and coordination of care. Among the key duties of this position are the following: Provides case management and community support services to assist individuals and/or families to gain access to needed medical, social, education, mental health, addiction, co-occurring and other services. Demonstrates the ability to engage and establish a working relationship with and provide services to individuals with physical health, mental health, and substance use disorders. Collaborates effectively with members of the department, and with external departments, staff, referral sources, other service providers and social support networks to provide services and rehabilitative care for individuals and families. Maintains appropriate records as mandated by University Behavioral Health Care policy and applicable accrediting organization standards. Demonstrates commitment to the mission and goals of the unit. Meets annual requirements.
