Having served the Trenton area for more than 45 years, Anchor House has been providing services to youth and young adults who have run away and those that are experiencing or are at risk of homelessness, abuse, or neglect. We are looking to add to our dynamic team! Our ideal candidate is looking for a career with purpose in an established organization that is big enough to offer work in different areas, but still small enough that you are known by your name and not just a number. We are currently seeking a Case Manager, for our Rapid Rehousing Program, to join our team. Our Case Managers ensure that each client is placed in the right housing and actively manage all case activities, coordinating support services to meet their needs and empower their success. This position is Monday - Friday from 9:00 a.m. to 5:00 p.m.
Salary Range:
$50,000 to $53,000
What You'll Do:
Secure housing for youth and develop individualized case management plans focusing on Life Skills, Education, and Housing. Supervise clients, conduct life skills activities and assessments, process referrals, handle donations, and manage transportation and house upkeep. Administer Housing Assessments and re-assess as needed. Provide support services and collaborate with landlords and housing resources to achieve client goals. Enter data into HMIS Software and generate monthly program reports. Offer direct care, supervision, and transportation for appointments and activities. Provide crisis intervention, individual/family/group counseling, and advocate for clients. Maintain client files, including case plans, progress notes, and transportation records..
What You Bring:
A Bachelor's Degree in Psychology, Social Work or related human services field 1+ years experience working with youth Prior housing experience preferred Ability to work well in a team setting Effective verbal and written communication skills
