Job Description

New Jersey Mentor , a part of the Sevita family , provides Residential and Treatment Foster Care services to youth who may be facing out-of-home placements, transitioning back into the family home or independent living alternatives, or involved in the juvenile justice system. Through our personalized programming, we help youth gain stability, comfort and confidence to overcome all challenges and live well. Laurel Service Area (Camden, Cumberland, Atlantic Counties) $45,000 Case Manager Everybody needs a job but only extraordinary people work here. Utilize Microsoft Windows programs to track and maintain data for multiple rolling deadlines related youth in care Communicate with external team partners including DCPP, CMO, and family members of youth Conduct home visits and office base services, and accompany individuals being served as needed Responsible for the annual contractual review process for Host Home Providers Maintain and audit files regularly for compliance Coordinate care with team members to ensure youth receive high-quality care Exceptional organization and time management practices required Flexible in daily schedule to best facilitate program needs Interface effectively with internal team members to ensure licensing regulations are met Performs other related duties and activities as required
Qualifications:
Bachelor's degree in human services. . Three years of related work experience and knowledge of case management. Valid driver's license, registration, and insurance. Ability to establish working relationships with individuals served. Demonstrated competence in verbal and written communication skills. Ability to handle crisis situations. Working knowledge of Excel Ability to learn other relevant computer systems Ability to work independently Why Join Us?
