• Why does the job exist? Work with
CYFD JJS
clients who are aged 18-21 to return to the community following a commitment to a secured JJS facility. Work is primarily done in the field, assisting clients with job search, locating housing, budgeting and life skills. How does it get done? Work is primarily done both in the field and the JJS secured facilities. Working with clients in the secured facilities includes but is not limited to attending monthly meetings, meeting clients in person and assisting in discharge planning. Working with clients in the field includes but is not limited to assisting clients with job search, locating housing, budgeting, and life skills. There may be occasions to transport clients in state vehicles to appointments ect. Work with the Multidisciplinary Team while JJS clients are completing their commitment to create their transition plan to return to the community. Once clients are placed on supervised release Transition Coordinators work one on one with clients to return to the community. Work also includes utilizing pay cards and completing end of the month financial paperwork as required. Who are the customers? CYFD Juvenile Justice Clients Ideal Candidate CYFD Juvenile Justice Clients Minimum Qualification Associate's Degree in Social Work, Psychology, Guidance and Counseling, Education, Sociology, Criminal Justice, Criminology, Family Studies/Services, Social Sciences, or Human Services and one (1) year of experience as a social work assistant and/or in social or community coordination, working with communities, working on health or social service related matters, social work/case management experience, and/or behavioral health care. Any combination of education from an accredited college or university in a related field and/or direct experience in this occupation totaling three (3) years may substitute for the required education and experience. Employment Requirements Must possess a valid Driver's License. Pre-employment criminal background check is required and conditional pending results. Working Conditions Work is conducted in a office setting as well as the field working with clients in the community. Must have a valid driver's license and be able to drive state vehicles. Exposure to Visual/Video Display Terminal, personal computer and telephone usage. May have periods of sitting. Work will be with potentially hostile/aggressive clients.
