Job Description

Posting Details Aging and Long-Term Services Department's (ALTSD) mission is the belief that New Mexico's older adults and adults with disabilities have the right to remain active participants in their communities, to age with respect and dignity, to be protected from abuse, neglect, and exploitation and to have equal access to health care. The Long-Term Care Division (LTCD) at ALTSD administer community-based programs to allow older adults to thrive in their homes and communities to reduce nursing home placements and emergency department visits. Interviews are anticipated to be conducted within two (2) weeks of the closing date. Applications submitted without work experience or a current resume will not be considered. Under the Direction of the Coordinator of LTC Programs, the Social Services Coordinator Kinship Caregiver Navigator will: Oversee program rules and eligibility Provide technical assistance and support to participants, caregivers, and contractors Resolve complex client issues Conduct in-home visits and complete assessments Track and report performance measures accurately and timely, as established by the agency Assist the program with other duties as assigned. How does it get done? At the direction of the Coordinator of LTC Programs, the Social Services Coordinator Kinship Caregiver Navigator is responsible for: Case Management-Recruit participants and determine eligibility Conduct program presentations Track and monitor participant cases, including care plans and follow-up Provide in-home visits and assessments Connect families with local resources (financial aid, legal support, healthcare, etc.) Make necessary referrals and provide support through both virtual and in-person engagement Maintain assigned caseloads. Outreach to Participants and coordination with partners. Collaborate with service providers, state agencies, and community organizations Advocate for services and assist families in accessing benefits Stay informed on federal, state, and local benefits and resources Maintain partnerships to support program goals
Tracking and Reporting:
Maintain accurate and timely case records. Who are the customers? ALTSD clients, other staff agency, contractors, community organizations. Ideal Candidate ALTSD clients, other staff agency, contractors, community organizations. Minimum Qualification High School diploma or equivalent and two (2) years of experience as a social work assistant and/or in social or community coordination, working with communities, working on health or social service related matters, social work/case management experience, and/or behavioral health care. Any combination of education from an accredited college or university in a related field and/or direct experience in this occupation totaling two (2) years may substitute for the required experience. Employment Requirements Must possess and maintain a valid Driver's License. Background and business reference check and Driver's License verification are required, and employment is conditional pending results. Must possess and maintain a current Defensive Driving Course certificate from the State of New Mexico or must pass and receive a Defensive Driving Course certificate as a condition of continued employment. Working Conditions Work is performed in an office setting and community setting with extensive use of the personal computer, telephone, fax,, copier and other related office equipment.
