JOB TITLE
LEAD Case Manager I, Raton/Clayton
REPORTS TO
Program Director:
Adult Services Position Status:
Regular part-time/full-time hourly non exempt employee at 26-36 hours/week This position provides intensive case management, which includes street outreach, and care coordination for individuals who have been referred by police officers to the Law Enforcement Assisted Diversion (LEAD) project. General Expectations Provide service with integrity, respect, empathy, and confidentiality Create a safe and structured environment to provide service that is trauma-informed
Education and Experience Requirements:
High school diploma or equivalent Minimum of 1 year of related work experience
Preferred:
Bachelor's degree in social work, psychology, or related field; or Community Health Worker or Peer Support Worker certification. Experience will be considered as a substitute for education.
Preferred:
Experience working with case management and working with individuals with substance use disorder. Primary Responsibilities Communicate effectively with Police Department staff, local DA offices and LEAD partners in Colfax and Union counties Conduct community based outreach, attend community health and education fairs about LEAD services in Clayton and Raton and outlining areas Develop an individualized care based plan with clients Monitor and update care plan through regular communication with Clinical Supervisor, PD, and case conferencing with other providers Advocate with appropriate agencies for services and assist participants in the scheduling and coordination of services Conduct field and home visits as appropriate Accompany participants to appointments as appropriate Maintain professional relationships and effective communication with the participating Police Departments, LEAD partners, and service providers in Raton and Clayton Maintain case records and documentation to meet agency standards Assist with compiling and reporting data for monthly and quarterly reports to funding sources and LEAD evaluation team Attend LEAD Operational Workgroup Meeting and other scheduled meetings Participate in regular reflective supervision with Program Director Complete trainings as assigned Participate in the on-call phone rotation schedule Maintain a valid driver's license and clean driving record based on DreamTree Project insurance requirements Participate in other duties as assigned
Additional Compensation:
$100 = 30 Days after start date $100 = 60 Days after start date
