40 Position Summary The Case Manager provides case management and academic services for SFCC students with disabilities. Coordinates with faculty and staff to ensure students receive appropriate accommodation and support. Monitors student progress and adjusts support plans as needed to address evolving needs. The Case Manager does not supervise others. Duties & Responsibilities Ensures students with disabilities receive appropriate accommodations and support to succeed academically by conducting confidential intake interviews, collecting disability documentation, and evaluating course placements while determining reasonable accommodations. Issues letters of accommodation and collaborates with faculty and staff. Maintains comprehensive student files and database records, including intake paperwork, accommodation requests, documentation of disability, and records of student meetings and actions taken. Interprets and applies ADA, Section 504, Title IX, FERPA, institutional policies, and procedures to guide accommodation decisions, protect student rights, and ensure compliance with federal and state regulations. Provides individualized case management for students as needs arise such as low grades, communication concerns with instructors, and the need for study skill development. Provides academic advising, facilitates registration and financial aid procedures, and makes referrals to appropriate college services and community resources. Advocates for accessibility and inclusivity by researching and recommending effective methods for procuring texts in alternative formats to support diverse learning needs and enhance educational resources for all students. Develops and executes a comprehensive technology plan for the department, researching and integrating best practices for adaptive technology. Encourages student utilization of these resources by promoting awareness of available adaptive technologies. Enhances the skills and knowledge of faculty and staff through continuous training and professional development, ensuring they are well-equipped to support students effectively and utilize best practices in their roles. Facilitates smooth transitions for high school students, parents, and faculty by devising comprehensive transition programs that prepare them for the academic and social challenges of college life. Creates and participates in presentations, meetings, and workshops to develop student support, including accommodation plans, disability services, and retention strategies. Knowledge, Skills, & Abilities Knowledge of Americans with Disabilities Act regulations. Knowledge of disability services principles and practices. Knowledge of case management principles. Knowledge of disabilities and related challenges. Knowledge of available assistive and adaptive technology. Knowledge of computers and job-related software programs. Knowledge of college policies and procedures. Knowledge of records management principles. Skill in developing and maintaining supportive and respectful relationships with individuals. Skill in decision making and problem solving. Skill in interpersonal relations and in dealing with the public. Skill in oral and written communication.Minimum Qualifications:
Bachelor's degree in Human Services, Behavioral Sciences, Social Sciences, or related field. Three (3) years of job related experience. Required education may be substituted for relevant experience (above the minimum required) for up to two years.Preferred Qualifications:
Bilingual (English/Spanish) Physical Demands The work is performed in a typical office environment. The employee occasionally lifts up to 25 lbs. Schedule Designation Typically 8:00 a.m. - 5:00 p.m., Monday through Friday. May be required to work afterhours and/or weekends.
