$58000 OBJECTIVE
AND POSITION SUMMARY
The Lead Case Manger is responsible for providing comprehensive case management and supportive services guidance Supportive Housing Services case management team. They will train and guide staff in providing case management utilizing trauma-informed care and a strength-based approach, which are in line with Housing First and harm reduction principles. In addition, maintain an active supportive service caseload, provide culturally inclusive, trauma informed services and will incorporate best practice techniques into the program. They will assist the Supportive Housing Program Manager with adherence standards. This includes assisting Case Managers in monitoring, reviewing and assisting executing service provisions based on the core components of the housing programs Scope of Work (SOW). Lead Case Manager will monitor and provide guidance to case managers related to housing programs.
SUPERVISORY RESPONSIBILITIES
Serve as the Technical Lead for HMIS, CareWare, and RWise, managing database oversight, staff data entry support, and ensuring accurate, timely, and compliant information First escalation for staff in handling difficult or complex cases Provide day-to-day assistance to the Supportive Housing Manager with supervision of Case Managers/Navigators, implementing all applicable program policies and procedures Guide staff in providing case management utilizing trauma informed care and a strengths-based approach, which are in line with Housing First and harm reduction principles Assist with financial requests for the Supportive Housing Programs Assist with unit inspections for the HOPWA and Operation Home Programs
SPECIALIZATIONS
Knowledge of
HIV/AIDS
issues. Proven experience serving the LGBTQ+ community, applying traumaâ€'informed and culturally competent approaches to reduce barriers and improve access to resources Knowledge of supportive housing programs and issues related to homelessness. Experience working with Clark County human service providers. Strong communication skills are critical, both verbally and in writing Must be able to work independently and as a part of a team Highly proficient in HMIS, CAREWare, and RWISE, ensuring accurate data entry, compliance reporting, and system integrity Ability to commit to teamwork and establish effective working relationships Experience in completing unit inspections using The National Standards for the Physical Inspection of Real Estate (NSPIRE), HOPWA HQS Habitability Standards, and the Carbon Monoxide Certification forms.
MINIMUM QUALIFICATIONS AND COMPETENCIES
Education/Background:
A combination of a minimum of (3) years of work experience and education will be considered. Bachelor's degree in social work, humanities, psychology, or counseling or other related field of study and one year of experience in social service delivery with LGBTQ+ community, individuals experiencing homelessness or at risk of homelessness, and/or those living with HIV, especially those experiencing housing instability Associate's degree in social work, Humanities, Psychology, or Counseling or other related field of study and two years of experience in social service delivery with LGBTQ+ community, individuals experiencing homelessness or at risk of homelessness, and/or those living with HIV, especially those experiencing housing instability High School diploma and three years of experience in social service delivery with LGBTQ+ community, individuals experiencing homelessness or at risk of homelessness, and/or those living with HIV, especially those experiencing housing instability Minimum of two years of handsâ€'on experience with HMIS, CareWare, and RWise database systems Bilingual, English and Spanish preferred Ability to establish and implement effective administrative programs and procedures. Ability to employ economic and accounting principles and practices in the analysis and reporting of financial data Knowledge of Microsoft Office applications (including Teams, Outlook, and Point) This position includes duties subject to ageâ€'based legal requirements. Therefore, candidates must be at least 21 years of age to perform essential job functions and to comply with federal and state regulations governing the work performed in this role. This position requires driving on behalf of the company. Candidates must hold a valid Nevada driver's license, meet the company's motor vehicle record (MVR) standards, meet company's insurance eligibility, and comply with all state and federal driving requirements. Ongoing employment is contingent upon maintaining an acceptable driving record. Other Requirements Experience and commitment to working with multiethnic, multicultural, low-income populations. Excellent attention to detail, analytical, and problem-solving skills. Demonstrated interpersonal and verbal and written communication skills. Proficient with Microsoft Office (Word, Excel, PowerPoint, Outlook, especially Microsoft teams). Excellent documentation and clinical skills. Proven ability to follow up and meet deadlines.
