Non-Exempt South Ferry House is seeking a compassionate and motivated Case Manager to provide direct support services to individuals experiencing homelessness in a 24-hour emergency shelter setting. The Case Manager works closely with guests to identify goals, connect to community resources, and build pathways toward long-term stability and self-sufficiency using a trauma-informed and person-centered approach. Responsibilities include conducting intakes and ongoing assessments, developing individualized service plans, assisting guests with housing and public benefits, and coordinating referrals to healthcare, behavioral health, legal, and social service providers. The Case Manager will also provide crisis intervention, advocacy, and ongoing support while maintaining accurate documentation in HMIS and other required systems. This role requires strong communication, organization, and relationship-building skills, along with the ability to work effectively in a fast-paced team environment. The Case Manager will contribute to maintaining a safe, respectful, and supportive shelter environment and may assist with daily shelter operations such as meal support, light cleaning, and guest transitions as needed. Qualified candidates must possess a high school diploma or equivalent and at least one year of experience working with vulnerable populations in a case management or direct service capacity. Knowledge of homelessness, mental health, substance use, and community resources is strongly preferred. A valid NYS driver's license and willingness to obtain CPR/First Aid and Narcan certification are required. Experience with crisis intervention, Housing First principles, and public benefit systems is preferred. Associate or Bachelor's degree in Human Services or a related field is a plus.
