Job Description

Title:
Intake and Service Coordinator Job Overview We are seeking a compassionate and trauma informed Intake and Service Coordinator to join our dedicated team. In this vital role, your expertise will help facilitate access to essential services and coordination of care . This position offers an exciting opportunity to make a meaningful difference in the lives of others.
Status:
Full Time (37.5) Hourly. Monday to Friday. Benefits Eligible.
Salary:
$27-28/hr. depending on experience
Qualifications :
1. Bachelor's degree in health or human services or an associate degree and equivalent training and experience working with populations impacted by homelessness, addictions, mental health conditions and/or other barriers to stable housing. 2. Experience working with people with HIV/AIDS and/or other chronic illness. 3. Professional interpersonal, verbal, and written communication skills and ability to interact with a wide range of personalities. An understanding of the social constructs that impact those struggling with homelessness and economic instability. 4. Effective communication, crisis intervention, and conflict resolution skills. 5. Able to work in and grow our agency culture, in a diverse and multicultural setting, providing trauma-informed and harm reduction services to persons of diverse backgrounds. 6. Time management and organizational skills with ability to manage multiple tasks and priorities. 7. Proficiency in Microsoft Office and Outlook. Familiarity with client services databases such as HMIS and AIRS a plus. The use of the agency Electronic Health Record database is also required. Valid, unrestricted driver's license and independent means of transportation Responsibilities 1. plans, and ensures internal and external referrals are made and completed to address medical re and barriers to the social determinants of health. 2. Coordinates with client's interdisciplinary care team for case conferencing and staffing changes and assists with client communication between case conferences. 3. Assists in the coordination of monthly case conferences with RW/AFPH (Alliance for Positive Health) and Albany Medical Center staff. 4. Coordinates with Peer Navigator III for peer support services for clients. 5. Enter all services in an online database (Electronic Health Records - EHR) as well as in AIRS (AIDS Institute Reporting System) and referrals in Unite Us within 48 hours of service. 6. Utilizes HIXNY (Health Information Exchange New York) for client medical visits and viral load proof. 7. Completes client's progress notes that follow the clients progress and needs. 8. Ensure compliance with Article 27F HIV Confidentiality Law and HIPAA confidentiality for all interactions. 9. Model and maintain appropriate professional boundaries with all members and staff. 10. Work to support members in reducing barriers to help each person achieve HIV Viral Load Suppression. 11. Attend full staff meetings and meet the required 24 hours of annual training. 12. Fully engage in self-care to promote physical, emotional, and spiritual well-being. 13. Other duties and special projects as assigned. Pay:
$27.00 - $28.00 per hour
