Job Summary:
As an employee providing direct services to the public (youth and their families who are at risk for law enforcement involvement, inpatient hospitalization, frequent use of the emergency department, loss of placement in the community, disruption of family placement and relationships), the HBCI Family Interventionist plays an important role in advocacy, safety planning, crisis intervention, skill building, information/referral services and community education/outreach. This position is considered full-time, salaried/exempt. The Interventionist responds to referrals for youth and families who are experiencing stress related to behavioral health challenges, provides assessments and initial safety planning, resource brokering, skill building, and on-call services to those appropriately referred to the program.
Qualifications:
Master's degree in a Human Service Field and license preferred, Bachelor's degree minimum required At least one year of professional experience with individuals in the community with behavioral health challenges; 3 years with Bachelor's degree General knowledge of mental health, substance use disorders, serious emotional disturbances. Ability to complete general assessments for individual and family functioning Must be able and willing to be on-call one week in 4 or 5; Sunday through Saturday 4pm to 8am or at arranged times. Provides referrals to local services; provides information/education about behavioral health challenges. Exceptional listening and engagement skills. Ability to remain calm during crisis situations. Ability to respond with empathy and support to individuals and families in crisis situations and with sensitivity and awareness to diverse cultural, ethnic and social backgrounds, values, and attitudes. Ability to assist individuals and families in developing and applying coping strategies Ability to work with families to develop appropriate goals, objectives, and action plans, including discharge criteria Willing and able to connect and follow through with community resources including convening and overseeing multi-disciplinary/multi-agency care coordination meetings Ability to make decisions within the guidelines of the agencies policies and procedures, federal and state laws. Knowledge of resources in the North Country region. Must pass NYS Criminal Justice Background Check, Not listed on the Staff Exclusion List and able to clear and OCFS Background Check Serves as role model of work and behavior standards for Community Connections in and outside of the workplace including social media profiles and blog.
Job Responsibilities:
Work in the community and family homes to complete assessments, create realistic safety plans, action plans Ability to work shifts which will provide coverage for during operational hours 24/7 hours, 365 days per year including holidays. Abide by all agency policies and procedure. Maintain accurate and timely documentation of services including all calls referred to 911 or crisis line. Complete and turn in time sheets at the end of the payroll period. Must complete agency mandated trainings as well as Question, Persuade, Refer (QRP), Information Security Awareness, Right-To-Know as well as HBCI specific trainings Demonstrated ability to manage high stress situations. Ability to work as a team member, providing support and constructive feedback in interpersonal interaction Any and all other job duties assigned by supervisor.
