Case Manager

JOB DESCRIPTION

Under the general supervision of the Youth Services Manager, the Case Manager (CM) provides "Hope Through Hospitality", advocacy, information, referral, crisis intervention services and short-term case management to clients residing in our Youth Shelter as well as providing some After Care services for community-based youth who elect to retain a Case Manager after they leave the Shelter. The CM will work with coworkers, supervisors and staff from other departments to ensure client needs are being met and to improve processes and services. Must be flexible with schedule to meet the needs of the clients which may include some evenings and weekends. Additional hours may be required to meet program deadlines or client needs.

QUALIFICATIONS FOR THE POSITION

(Not the Individual Qualification) 1. Bachelor's Degree in Health and Human Services or related field. 2. 3-5 years' experience working with at risk or homeless Youth 3. Ability to relate to and work with ill, disabled, elderly, emotionally upset and at times hostile people. 4. Integrity, positive attitude, mission-driven, and self-directed with demonstrated passion for HONOR's mission. 5. Must have and maintain a valid New York State driver's license 6. Detail-oriented and have competent literacy and writing, documentation, communications and interpersonal abilities (friendly, courteous, helpful, ability to work as part of a team). 7. Bilingual preferred but not required. 8. SOAR Certified or able to become certified within a reasonable amount of time.
