Qualifications:
Bachelor's Degree in Human Services, Case Management, Social Work, or a related field. Previous experience in case management within assisted living or a similar healthcare environment. Strong interpersonal, communication, and advocacy skills with the ability to build rapport across all levels. Proficiency in electronic health record (EHR) systems and standard office software. Ability to work independently, exercise sound judgment, and collaborate effectively with interdisciplinary teams
