This role is centered on helping individuals navigate the challenges that can come with involvement in the justice system. Whether connecting clients to community resources, providing support through court-related processes, or advocating for their needs, the Case Manager plays an important role in helping people move forward and achieve greater stability. We are seeking someone who enjoys working directly with people, building relationships, solving problems, and collaborating with community partners. The ideal candidate is a strong communicator who can balance empathy with professionalism and is committed to promoting fairness, dignity, and access to resources for all individuals. Responsibilities Meet with clients to assess their needs, goals, and challenges. Develop individualized plans that connect clients with appropriate services and resources. Provide guidance and support to individuals involved in the justice system and help them understand available options and services. Coordinate referrals for housing, employment assistance, mental health services, substance use treatment, education, transportation, and other community supports. Maintain accurate and timely case notes, records, and documentation. Build and maintain working relationships with courts, attorneys, probation officers, social service agencies, and community organizations. Monitor client progress and provide ongoing support and follow-up as needed. Advocate on behalf of clients while encouraging personal responsibility and self-sufficiency. Participate in team meetings, community outreach activities, and program initiatives. Ensure all services are provided in accordance with agency policies and confidentiality standards. Qualifications Experience in case management, advocacy, social services, community outreach, or a related field is preferred. Knowledge of the criminal justice system and community resources is a plus. Strong communication, interpersonal, and organizational skills. Ability to work effectively with individuals from diverse backgrounds and experiences. Experience with documentation, data entry, and case management systems. Ability to manage multiple priorities while maintaining attention to detail. Commitment to treating all clients with respect, empathy, and professionalism. Why Join Our Team?
