Full Job Description SUMMARY:
Join JFS Rochester in making a meaningful impact in the lives of individuals and families experiencing housing instability and homelessness. The Housing Stability Care Manager serves as a vital connection between food access, housing support, and community resources, helping clients navigate barriers and achieve long-term stability. Based primarily at the Brighton Food Cupboard, this role combines outreach, housing navigation, care management, and community collaboration to support vulnerable households through a trauma-informed, person-centered approach. We are seeking a compassionate, resourceful professional who is passionate about building relationships, advocating for others, and creating pathways to stability and well-being. KEY RESPONSIBILITIES
Identify and engage individuals and families experiencing or at risk of homelessness or housing instability through on-site presence and community outreach. Conduct comprehensive housing and psychosocial assessments to evaluate client needs, strengths, and service eligibility. Assess and verify program eligibility and maintain compliance with applicable agency, funder, and program requirements. Develop and implement individualized service plans that support housing stability and overall well-being. Provide housing-focused case management and homelessness prevention services, including diversion/problem-solving, housing search support, landlord engagement, benefits navigation, and connection to community resources. Build and maintain relationships with landlords, housing providers, and community partners to expand housing opportunities and service access for clients. Advocate on behalf of clients with landlords, service providers, and relevant systems. Provide ongoing follow-up and support to help clients maintain housing stability, including landlord mediation and problem-solving around tenancy issues. Facilitate referrals to mental health services through the JFS Counseling Center and coordinate care as appropriate. Collaborate with internal JFS programs and external housing, healthcare, and community partners to ensure continuity and effectiveness of services. Track client outcomes and program metrics; assist with data collection and reporting to support program evaluation and funder requirements. Must be highly mobile and able to travel regularly throughout Monroe County to engage and collaborate with partner food pantries and community sites. Maintain accurate, timely, and confidential documentation in HMIS and other required systems in compliance with agency policies and funder requirements. Participate in team meetings, case reviews, and required training. Uphold JFS values, including dignity, respect, equity, cultural responsiveness, and trauma-informed, person-centered care. Bachelor's degree in social work, Human Services, or a related field preferred; OR an associate's degree in human services or a related field with at least two years of full-time experience working with individuals experiencing homelessness or other underserved populations. Knowledge of housing resources, public benefits, and community-based services. Strong assessment, engagement, and case management skills. Proficiency with computer systems, including Microsoft Office 365 (Outlook, Word, Excel); experience with electronic medical records (EMR) or comparable case management/housing database systems (e.g., HMIS) preferred. Ability to work both independently and collaboratively in a fast-paced, community-based environment. Excellent communication, organizational, and documentation skills. Demonstrated understanding and application of trauma-informed care principles. SALARY & BENEFITS
