Job Description

Job Summary Full-time The Service Coordinator (SNC) coordinates services and provides advocacy, support, and linkage for an assigned caseload of individuals who are receiving community-based services as an alternative to institutionalization in a nursing facility. The primary purpose of this position is to support participants in the
NHTD/ TBI
Waivers to live in the community.Job Duties:
Develop individual service plans in conjunction with assigned caseload and maintain those plans to ensure all timelines are followed and the quality of services are being given. Link the participants with an appropriate service provider who can assist them in implementing the appropriate portion of their service plan. Assist the participant in securing a community residency that meets their individual welfare and safety needs. Participate in team meetings with participants and their service providers, and advocate for their rights and implementation of needed and desired services. Attend appointments and meetings with participants as needed. Conduct monthly contact and quarterly home visits with assigned participants, and maintain communication with participants' service providers and families. Contemporaneously and accurately document all services in the organization's ILS documentation system. Maintain required files. Ensure all documentation is up to date and accurate for each participant in accordance with the record-keeping requirements. Ensure that all Incident Reporting requirements are updated and distributed as needed. Attend and participate in professional development trainings as needed, and review program updates as administered to ensure all regulatory changes are being implemented. Be committed to all that you do to support and model the CDR Mission. Provide scheduled on-call assistance for program participants. Write blog posts and news letter articles on program activities. Other duties as assigned.Qualifications:
Bachelor's degree and one year of experience providing case management/ service coordination and information, linkages, and referrals regarding community-based services for individuals with disabilities.Skills and Competencies:
Excellent organizational skills and attention to detail. Strong interpersonal, negotiation, and conflict resolution skills. Good interpersonal and written communication skills. Impeccable time management skills with a proven ability to be punctual and meet deadlines. Commitment to the mission and work of CDR.
