Advocate/Case Manager

The Saratoga Senior Center, located in the heart of downtown Saratoga, serves over 4,000 older adults by providing a supportive and engaging community environment. The center offers a variety of programs, including healthy living classes, skill-building activities, and social opportunities that promote physical and mental well-being. Additionally, the Center provides aging in place assistance and supports seniors in maintaining independence through essential services like advocacy, food assistance, transportation, a volunteer match program and its palliative care based Senior Life Transitions program, which provides vital help to seriously ill older adults. The Advocate/Care Manager will play a vital role in providing advocacy, resources, and support to seniors to ensure they maintain their independence and well-being. Daily tasks include assessing client needs, developing individualized care plans, coordinating access to services, and providing referrals to appropriate programs or resources. The role also involves working closely with families, healthcare providers, and community organizations. This is a full-time, on-site position located in Saratoga Springs, NY.
