Care Manager Status:
Full Time, Non-Exempt Reports to: Associate Executive Director We're looking for someone who wants to make a difference. We're looking for a case manager to help us support people experiencing homelessness at our shelter in downtown Saratoga Springs. Case managers work directly with our guests and in partnership with them develop plans that will lead to lives with purpose, dignity and hope. The Case Manager will be expected to develop relationships with our guests, model positive life skills, assess strengths each guests possesses and develop case plans to build on them. Knowledge of community resources is a big plus. Good boundaries, non-judgmental positive attitude an absolute requirement. Duties and responsibilities Responsible for completing intakes on all guests when on site or review off-hour intakes within the next business day Maintains current guest documentation and develops incident reports as needed, which will be reviewed by the Program Director, to ensure compliance with state guidelines Meet weekly with guests on caseload to establish or update goals and document interactions. Secures supporting documentation necessary for guest's progress towards achieving and sustaining goals Educate and refer guests to community agencies that appropriately address guest needs for medical or mental health support, housing, employment, transportation, etc. Must be knowledgeable about local service providers and how to access necessary services and benefits Act as an advocate and work cooperatively with other human service organizations to improve access to needed services Assess caseload and engage time management strategies, such as utilizing volunteer and intern support in assisting with appropriate tasks Providing oversight of guest non-compliance and working with guests to ensure adherence to house rules and ADLS Collect rent from houseguests and provide receipts as necessary Reconciles house guest rents with finance on a weekly basis Conducts breathalyzers and drug screenings as necessary Provide transportation assistance to houseguests when applicable Contribute ideas and efforts to the constant improvement of SOS services Attend agency, team and community meetings as assigned Utilize shared Calendar keeping schedule updated for guests appointments Use organizational data sharing methods, including Google Drive and Dropbox Other duties as assigned Required Skills and Attributes Ability to develop trusting relationships with people who have difficulty establishing trusting relationships Ability to function as a positive member of a team Understanding of the link between poverty, and homelessness Knowledge of trauma informed care preferred Basic knowledge of local human service agencies and organizations. Ability to work well independently Creative problem-solving and crisis management skills Ability to exercise good judgment and discretion. Excellent verbal and written communication skills. Physical Requirements Position requires ability to life, carry up to 10 pounds unassisted. Must be able to climb stairs. Must be able to squat, bend and do overhead reaching and lifting. Minimum Qualifications, Education, Training and Experience Bachelor's degree in Social Work, Human Services or other related field or Associate's degree with 2 years experience. Education can be subbed for experience. Valid driver's license and clean driving record required. Proficient in use of applicable computer programs (Microsoft Word, e-mail, and web based HMIS software).
