The HRSN Case Manager provides direct, person-centered case management services aligned with Healthy Alliance HRSN service categories. This role focuses on addressing housing instability, economic hardship, and related social needs through assessment, service coordination, referrals, and follow-up to promote long-term stability and self-sufficiency. Key Responsibilities Conduct HRSN screenings, intake assessments, and individualized service planning Manage an active caseload of clients receiving housing and economic stability supports Assist clients with housing applications, inspections, leases, and move-in processes Coordinate rental assistance, CTS services, utilities, transportation, and emergency needs Provide economic stability support, including benefits navigation and employment readiness referrals Maintain accurate, timely documentation and data entry per Healthy Alliance requirements Collaborate with landlords, housing providers, and community partners Participate in supervision, team meetings, and required trainings Deliver services using trauma-informed and strengths-based approaches This position offers an exciting opportunity to make a meaningful impact in the lives of those facing housing challenges through compassionate support and strategic intervention. Requirements Associates degree in Human Services, Social Work, or related field required Experience in housing navigation, case management, or community-based services preferred Familiarity with HRSN and housing stabilization models preferred Strong documentation and time-management skills
