Job Description

Inspire of McClelland provides comfort of home to independent elders, with leisurely access to delicious & nutritious meals, wellness & social activities, assistance with tasks of daily living, and medication assistance seeks a Case Manager for immediate hire. We are looking for a positive, dependable, caring individual to join our team to meet the growing demand in our community for quality care. The Case Manager position requires the following: o Ability to communicate in a dignified, respectful manner. o Relates well to residents, visitors and other staff members o Emotional and physical stamina to be of service to the residents in their daily living skills o Interest and participation in opportunities for personal academic growth o Ability to practice self-control in speech, listening, body and facial mannerisms o Technologically inclined and have a working knowledge of Microsoft Office suite. o Possess physical endurance for frequent activity.
Responsibilities:
Make daily rounds to assess/monitor general condition of all residents. Provide general staff supervision to ensure the provision of quality care to residents Assist in staff orientation and staff scheduling. Assist in staff scheduled meetings as necessary. Work together with Medication Staff to check medication inventory and oversee the safe storage and disposal of medications. Liaise for residents between physicians, community health agencies, social agencies, family members and all other significant people. Work with together with residents to resolve concerns and issues. Document all significant incidents pertaining to resident condition. Review all appointment forms for doctor visits when resident returns from the doctor/clinic. Adjust medication changes/orders in resident charts and on the medication administration record. Assist with residents, charts and orders when the House Doctor and House Psychiatrist come to the home. Provide accurate and up-to-date documentation of Residents' Care Management needs (problems identified) as well as Case Management of intervention taken to address given problems Document and submit all incidents pertaining to Resident conditions as well as staff and visitors. Visit hospitals, private homes and long-term care facilities to assess and evaluate prospective Residents. Quality Assurance - evaluate all current intervention and make sure that such intervention measures of care are closely monitored and documented.
Qualifications:
Must be at least 18 years of age. An Associate Degree and three years of full-time experience in the provision of services to a dependent adult population OR a Bachelor's Degree and one year of full-time experience in the provision of services to a dependent adult population Prefer previous experience, either professional, personal or volunteer, with the elderly population. Must be proficient in
MS Suite Compensation:
